IMO it depends on what combat you're trying to pk at honestly, in the long run you probably should strive to get 45 defense eventually.

Still leveling my zerk, should I stay at 42 or get 45? Are the fremennik helms worth it or is a dyed rune full helm enough?

 

I tried 42 for a bit, couldn't be without the zerk helm. It didn't feel right!

45 def allows archer/mage/melee helms which are all useful for pvp/pvm and all 3 options are cheap
